Family Case Management
- Family Case Management is funded by the Illinois Department of Human Services (IDHS)
- The program provides public health nursing services to all medically
eligible pregnant women and children up to the age of 12 months. These services include home visits with an emphasis on prevention. Health
education, growth and development assessments, and referrals to needed services are provided. - The goal of the program is to improve the health of pregnant women and their children.
